PLAN YOUR VISIT:

Please plan to arrive 20 minutes prior to your scheduled appointment in order to complete the necessary paperwork. This will allow us to establish your medical file before you see the doctor.

PLEASE REMEMBER TO BRING THE FOLLOWING:

  • Your Insurance card
  • Identification: Copy of passport, or driving license, or labour card, or Emirates ID card.
  • Referral letter from your doctor (if you are referred to Dr Das by other doctor/ hospital)
  • Current and previous medication details, a copy of your X Rays / CT scans if any (not just the reports - please get the actual films)
  • Please be prepared to pay your Co-payments, unmet deductibles, and payment for services not covered by your health insurance. For your convenience, we accept all major credit cards.

HEALTH INSURANCE PLANS:

There are many different types of health insurance coverage. Your insurance coverage depend on the contract between you and your insurance company and whether or not JTS Medical Centre has a contract with your insurance company. Please visit the Medical Centre websites or call to check for Insurance coverage.
